Description
Further to Ticket 355 (Triplicate Options Columns)...
Sorry, I was unable to comment on that (closed) ticket, as I was rejected as possible spam.
Sorry, I know it is probably bad etiquette for me to:
(1) add this to end of someone else's post;
(2) especially as I have a slightly different configuration than OP;
(3) and make comments after ticket is closed, prior to trying soon to be released fix;
(4) and not having tried any of the fixes you suggested to the OP.
(5) Also, I'm writing anonymously, without an email.
(6) Finally, my post is verbose (though I hope that is helpful, rather than a problem).
So. Not a good start!
However, I thought you might like to know that I have very similar symptoms to the OP, judging from the screenshots. But I use Firefox, not Thunderbird.

SYMPTOMS & TROUBLESHOOTING CLUES
Note the weird headings I have seem to be linked to the "prefs" JScript Script File in this location: C:\Documents and Settings\[USERNAME]\Application Data\Mozilla\Firefox\Profiles\lxhpu61s.default
The headings read from left to right:
(1) Mozilla Firefox
(2) Chanelle Hayes - Magazine Shoots/007(4) - Mozilla Firefox (a bit unfortunate, that one!)
(3) Dell Ireland

Hope this helps, rather than hinders. Hope it doesn't add to headaches.
Thanks a million for developing and maintaining this extension for your many quietly grateful users.
